New Patient Registration
If you are in the practice area but still need to register with us please do so – and preferably before you actually need us, as it can take a month or two for your records to get to us from your previous surgery.
Patients registering with the Practice are asked to produce a form of identification, such as a passport or driving licence, and also documentary evidence of their address such as a recent utility bill showing both the name of the patient registering and their address. You can download the registration pack but will need to present at the surgery with this and your ID.
If you do have your medical card, name of your previous surgery and/or details of any current treatments that would be useful, but if not, never mind, our receptionists will help.
If you need to see a doctor or nurse the same day you register that's fine but please arrive a little early so we have time to sort the paperwork.
You are registered at a Group Practice. You have the right to express a preference of GP and whilst we try to comply with this it may not always be possible and in which case we will inform you of this.
